How to become a patient PATIENT RECOMMENDATION SAMPLE FORM to take to your own physician
Any MD.(medical Doctor) Or DO. (Doctor of osteopath) can sign a recommendation. You do not need a specialist or a “cannabis” doctor. It is always advisable that your primary care doctor is fully informed of your medical choices and monitors and provides for your follow up care!
